Default health insurance question?

I am trying to pick out a plan. I was wondering about co-pay for office visits, it says its a flat rate the selection is for $35. What if I do not select a co pay, would the rate vary or would it be much more expensive?

Also is a lower deductible always better? I know it increases monthly payments but I may have to have nose surgery because I can't breathe through my nose, so if I have $1000 deductible I would have to pay $1,000 before my insurance pays anything right?
