Default are orthotics normally covered by health insurance?

I'm having some lingering foot issues and I'm pretty sure I need orthotics. I'm trying to figure out if insurance companies normally cover them. I have United Healthcare PPO.

Has anybody ever got orthotics? If so, where did you get them from and were they covered by insurance? Any info will be helpful. Thanks!
I know it depends on what insurance company you have, but many things are GENERALLY covered by all or most insurance, i.e. if you break your leg.

For instance, propecia, the hairloss drug isn't covered by ANY insurance company.

I'm just looking for a general answer. Thanks!
