Depends on how extensive of a physical you need. Do you need blood work?? Urine sample?? Or just a doctor to check range of motion, and review your history??

You can usually get free TB tests at your local county health department. You might be able to get a physical, or find info on a clinic, at the county health department in your area.

there are many definitions of 'physical exam'. If it is within a school realm usually it is less expensive. Some are very thorough and some are just 15 min heart/lung ear and eye checks so its difficult to say what your charge will be. Cant you get an inexpensive policy with your school?
