Default Question about business ethics on Ebay + post office insurance question?

I don't know who else to ask - I sold an item on ebay and the woman told me the item came broken. I don't offer insurance, but she specifically insisted on insurance. Two days later, sure enough, her item is broken and she wishes her money back. Maybe this is just me, but it sounds fishy. Sure - I take her word for it and issue her a refund and collect damage insurance from the post office. What if the item isn't broken? Should I insist on getting my item back? I wouldn't take a damaged item back to a store, get my money back, but keep the item. Does anyone have any advice?

Also - I've never collected for the shipping insurance before- who collects? me, the seller? or her - the buyer? either way, it can work out, whether she gets her refund from the post office or i do and i send it to her. Does she have to present the item at the post office? I'm very confused and I really hope I'm not getting scammed. It's not worth it.
